Your reporting package includes aggregate participation results and health assessment responses to further refine your health and well-being strategies.
Examples include:
- Status Report — provides information that demonstrates the level of activity and involvement in programs. It presents enrollments, completions, and exemptions in each qualifying program.
- Health Assessment Summary Report — provides aggregate data on the results of health assessment responses provided by participants. The report includes information on prevalence of health issues and readiness to change among the population as well as estimated productivity impacts due to the health issues reported by the population.
- If the health assessment was completed in multiple years, this report also contains an addendum summarizing the change in each of the risk factors over time for both the entire population of health assessment completers and the subset of completers who took the health assessment in all years.
