I was extremely impressed with my coaches. I loved the fact that they were able to summarize my thoughts and helped me put them into goals. They understood me.
HealthPartners has a participant-centered health coaching model. The approach is tailored to an individual’s needs and goals with:
- Unlimited access to coaches
- Flexible scheduling
- Focus on the participant
Our health coaches have access to the participant’s health assessment responses and their HealthPotentialSM Report. This allows the health coach to consider the participant’s entire risk profile and support them in making informed decisions about which programs would be most beneficial for them to support sustainable lifestyle changes.
Coaching is designed to help participants develop a personal action plan and set realistic goals to improve overall health. Ongoing coaching focuses on progress toward meeting personal goals. Frequent check-ins help to keep participants motivated to make sustainable lifestyle changes.
Our health coaches are experienced health specialists. All health coaches are bachelor degree-educated or above. Many health coaches have a master’s degree in an allied health professional field of study or have certifications in specialized areas.
In addition, all health coaches are trained and have expertise in the following evidence-based behavior change methodologies:
- Intrinsic coaching — a methodology and approach to coaching that increases people's capacity to think better about choices — in ways that last — and also multiply from person to person
- The Transtheoretical Model (includes readiness to change construct)
- Motivational interviewing
- Relapse prevention
- The 5As (Assess, Advise, Agree, Assist, Arrange)
- Decision Support
